Child Protection Psychological Evaluations

The Judicial Branch assumed the responsibility for creating and maintaining a roster of and credentialing psychologists who conduct court-ordered evaluations in child protection cases effective January 1, 2010.

All Providers seeking to be included on the roster of evaluators are required to submit to a credentialing process.

As part of the credentialing process, each provider is required to complete and return the Provider Information Form.

The completed form should be sent to:
