The Supplemental Affidavit
(Form 1S) may only be used with the
permission of the Committee (it is password
protected) by applicants who must update an already-filed bar
application. The Supplemental Affidavit is not an application
for admission to the bar.

If you believe you
should be filing a Supplemental Affidavit you must contact the
Committee for the required password.
In addition to Form
1S you must file Form 5S. You
may also have to file Forms 2S, 3S, 8S, 10S and/or 11S.
